Signed Employment Contract
â–ª English version only.
â–ª All pages must be signed by both employer and employee. -
Passport – bio and signature, pages 2 and 3. Should be valid for at least 6 months.

Valid Residence Document
Submit scanned copy (front and back) of any of the following:
- Temporary Residence Card
- Employee Card
- Long-Term Residence Permit
- Karta Pobytu (Poland) -
Supporting Document
Either of the following:
- Addendum to the Employment Contract
- OFW/Migrant Workers Insurance (Policy/Certificate)

Addendum to the Employment Contract of Filipino Truck/Bus Drivers.
