Verification of BM Employment Contract-Process and Requirements 

Kasalukuyan ka bang nag ta-trabaho sa Czech Republic, Poland, Latvia, Lithuania o Estonia at BABALIK sa parehong kumpanya/employer pagkatapos ng iyong bakasyon? Kung OO, basahin ang mga sumusunod:

 

The following are the requirements for contract verification for balik-mangagawa who are currently working and will return to the SAME EMPLOYER after his/her vacation.

​

  1. Employment contract in English signed by employer and worker; Please indicate the date when the contract was signed.

  2. Addendum to EC-Template both signed by employer and worker.  If the employer is not amenable to sign said addendum, worker must submit OFW Compulsory Insurance which includes repatriation of frail body or human remains in case of disability or death. If you do not have an existing OFW Compulsory Insurance, kindly search for licensed Philippine insurance companies e.g. Pioneer, Paramount, Mapfre etc. which offer such products.  Some even allow online registration and payment. Provided below are links of accredited providers: ​

     For Filipino Truck/Bus drivers, kindly submit signed Addendum to Employment Contract of Filipino Truck/Bus               Drivers. Link: Addendum Filipino Truck/Bus Driver​

   3. Copy of passport bio and Signature page (pages 2 & 3) which has validity of at least 6 months.

​

   4. Copy of valid residence card, long term residence permit, employee card or karta pobytu.

 

Para sa mga nasa Latvia, Lithuania at Estonia, mag submit ng kopya ng PAYSLIP o screen shot ng bank transfer ng huling natanggap na sweldo kung saan makikita ang pangalan ng kumpanya o SIGNED (pirmadong) Certificate of Employment mula sa kumpanya na nakasaad ng DATE kung kailan nagsimulang mag trabaho sa naturang kumpanya. For those in Latvia, Lithuania and Estonia, submit a copy of the PAYSLIP or screen shot of the bank transfer of the last salary received which shows the name of the company or a SIGNED (signed) Certificate of Employment from the company stating the DATE when you started working for that company.

  • Please scan and submit your Employment Contract, the bio and signature pages of your passport (pages 2 and 3), any applicable addendum, your OFW Migrant Workers Insurance (Policy/Certificate), and for Truck/Bus drivers, the specific addendum for Filipino Truck/Bus drivers.

  • Document Quality: Must be clear, complete, and readable

The verification fee is worth CZK 250.00 (for those in the Czech Republic) or EUR10.00 (for those in Poland, Estonia, Latvia and Lithuania).

​

Ang isang balik manggagawa (vacationing OFW) ay yung BABALIK sa SAME EMPLOYER. Kung HINDI sa SAME EMPLOYER babalik o kung hindi pa nakakapag simulang mag trabaho sa kumpanya ay kailangan na kumuha ng OEC sa DMW bilang NEW HIRE worker.
